I authorize investigation of all statements contained herein and the references and employers listed above to give you any and all information concerning my previous employment and any pertinent information they may have, personal or otherwise, and release the company from all liability for any damage that may result from utilization of such information. I understand and agree that, if hired, my employment will be “at will,” which means employment is for an indefinite period of time and may be terminated by myself or National Pools at any time, with or without cause, and with or without notice. I authorize National Pools to obtain personal information contained in my Motor Vehicle Report. I understand that this information may be procured by an authorized representative of National Pools, including employees of National Pools’ insurance provider, and may include, for example, my photograph, name, address, telephone number, social security number, driver identification number, medial or disability information, and driving record. By signing this disclosure, I hereby authorize National Pools to procure such driver reports and additional driver reports about me from time to time, as it deems appropriate, to evaluate my insurability, employability, and for other permissible purposes. I understand that as a condition of employment I may be required to undergo and successfully pass an alcohol and drug screening. I also understand that, if employed, I may be required to submit to an alcohol and drug screening at any time in the Company’s discretion. I understand this application will be given careful consideration, and that acceptance of a completed form does not imply or guarantee employment.
